How is the score calculated?

Each graded question counts once, based on which answer you pick. The score is simply the share of those answers that landed in the "strong" bucket. A couple of context questions (like whether anyone depends on you financially) aren't scored — they just tailor later questions.

Why do the questions change by country?

Currency, government health schemes, and terms like "nomination" vs. "beneficiary designation" vary by country. Picking your country swaps in the right terms so the questions read naturally, without changing what's actually being asked.
